Acknowledgement of risk

I acknowledge that participation in activities at Activity Zone Bookings carries inherent risks including, but not limited to, slips, trips, falls, and minor injuries arising from physical play, climbing equipment, craft materials, and interaction with other participants.

Supervision & behaviour

I understand that while staff supervise activities, primary responsibility for my child's behaviour remains with me as the parent or guardian. I agree to remain on-site for the duration of the session unless my child is aged 6 or over and I have pre-arranged drop-off in writing.

Medical consent

In the event of injury or medical emergency, I authorise staff to administer basic first aid and, if necessary, to call emergency services.

Photography

Staff occasionally take photographs of activities for our website and social media. You may opt out below — if you do, we'll make sure your child isn't included in any images we share.

Data protection

Your information is stored securely in line with UK GDPR. We'll keep waiver records for 3 years from the date of your last session, after which they're automatically deleted. You can request a copy or deletion at any time by emailing info@activityzonebookings.co.uk.

Release of liability

I release Activity Zone Bookings, its staff, and its directors from liability for any injury, loss, or damage arising from normal participation in activities, except where such injury results from gross negligence.
